How to Read This Matrix

  • Responsible (R): The doer — the person or team who performs the work.
  • Accountable (A): The owner — one and only one per row. Answerable for the outcome.
  • Consulted (C): Two-way dialogue — input is sought before decisions are made.
  • Informed (I): One-way notification — kept in the loop after decisions or actions.

Single Accountability

Only one “A” per row. If two people are accountable, no one is.

Clear Consultation

“C” is bidirectional discussion before action — not a rubber stamp.

Timely Information

“I” is one-way notification, delivered when it enables action, not after.
